What is required to be an Individual Provider (IP)?

Requirements include:

• Being 18 years or older.
• Legally able to work in the United States.
• Completion of a Background Check and Required Training (Choose from the categories at the left to know more)
• Not being named on any of the Medicaid exclusion lists.
