Individual Providers (IP) and Clients will need their Person ID number to directly reach their assigned Service
Coordinator.

You can find your Person ID in the DirectMyCare 门户网站

• After logging in, click on your name in the upper right-hand corner of the screen.

• Your Person ID is located in the “Basic Information” section.

Remember: If you have questions about your recent pay amounts or pay history, our self-serve payroll
information system is always available. Call 866-214-9899 and press option #1.
