How can IPs provide their 10-digit confirmation code to CDWA?

Make sure to write the code down! There is no way to get the code after you have exited from the online form. You can provide the code to CDWA by:

• Listing this email address when filling out the background check authorization form: CDWABackgroundCheck@ConsumerDirectCare.com. The code will be sent directly to CDWA using this method.
• You can save/print the confirmation code and call, email, or provide the code in person to CDWA.
